For patients that are visiting from overseas, we would need a copy of your passport and any valid travel insurance certificate.

Please note that it is the policy of the hospital take a deposit from non residents until such

time as we have a payment guarantee from your insurer.

Additional Fees: 

 

There is an additional $20 dollar fee for completion of forms for use outside of the patient's medical record, including:

  • Day Care

  • Pre-Employment

  • Employment

  • Life Insurance.

  • Family Medical Leave
